Abstract

The obesity epidemic continues to rise. The most prevalent endocrine disease in obesity is subclinical primary hypothyroidism. Testing of all obese patients for the presence of impaired thyroid function is suggested. It is unclear whether impaired thyroid function is the cause or consequence of increased adiposity. There are also no clear data on the best way to doce levothyroxine (LT4) in patients with hypothyroidism and obesity, and the effect of bariatric surgery. All these points have been studied in this present doctoral thesis. After its completion, we concluded that, in patients with morbid obesity, thyroid-stimulating hormone (TSH) increases moderately and morbid obesity presents mild central resistance to thyroid hormones, these alterations are reversible with weight loss. The results of this thesis strongly suggest that LT4 dosing in obese hypothyroid patients can be individually tailored more precisely if based on ideal body weight.
