Abstract

Introduction: the mandibular skeletal Class II malocclusion represents one of the biggest challenges in today´s orthodontics, such the subjectivity of diagnosis as the limitation and complexity of treatment. In a few malocclusions there is an obvious disparity therapeutic criteria, ranging from the functionalist effort to stimulate mandibular growth resignation of orthognathic surgery or dentoalveolar camouflage. In this article we propose a new approach to the diagnosis and treatment of this malocclusion, focusing our approach on three key concepts: ecological orthodontics, as use of natural resources and muscles forces, ligaments and patient’s ATM, periodontal proprioception as a peripherals Petrovic oclusal homeostasis, «jumping the bite» and the concentration of treatment at the peak of pubertal growth. In this article we present our new protocols for diagnosis and treatment, chronobiology and biomechanics sequence as well as equipment designed to be performed (Class II Orthoflex® line), showing the most paradigmatic clinical cases with good and stable long-term results. We believe that the therapeutic option that we are exposing must be taken into account in the sagittal mandibular Class II problems, especially when the alternative proposals in contemporary orthodontics resignation goes through camouflage or complex orthognathic surgery.
